If you are going to optimize your website these thing should be considered...

1. In Title: you should insert your targeted keyword that makes a sentence.

2. In Description: Your web description should not longer than 250 characters and try to put your targeted keyword in fist line of description.

3. In Keyword: put your all keywords in keyword mete tag and don’t repeat any keywords

It will help you to increase your website ranking in search engine results.

A great tool I've used in the past is the google "Keyword tool" in AdSense. Helps you see what people are searching for and how many advertisers there are.

That's only in-page optimization.

You should also try off-page optimization.:

Build back links by submitting to search engines and directories, submitting articles and buying some quality links.

Hey keyword selection should be done smartly. Don't do keyword stuffing. Its of no use.
And for keyword selection you may use keyword selection tool like keywordDiscovery , there are many more available.
Hey anyone will tell me the possible reasons for the downfall of a webpage from google ranking being the page is properly optimized with good backlinks.

There are so many things to consider when working on SEO, I have done work for many sites by changing there entire SEO, the hardest ones are computer, gaming and health sites. I have worked on several hard sites including this viagra one (http://www.edguider.com) and let me tell you it is very hard to rank these type of sites. The best tool in my opinion is the keyword tool at tools.seobook.com (http://tools.seobook.com/) they provide a firefox plugin and keyword research tool better than the adwords one :)
