Default Using a form to generate a URL

I simply can't get my head around this, I want to use a simple for to generate a URL ... as a basic example: The user would input "andrew" in a form. A resulting web page would show a clickable URL (example: "www.mydomain.com/andrew") I need to work some validation around which "mydomain" URL to...

I'm pretty sure this is related to php or javascript. I'm not expert on either, but since you've gotten no response I though I'd give you an idea of where to start looking. W3 Schools has this somewhere in the php/java tutorials. Although I cannot remember exactly where it was I just read over it. Hope this helps a little bit.
